All of the forms that are used on this web site can be filled out using Adobe Reader.
If you have a MAC OS or a Windows computer, you can download the latest
version at
https://get.adobe.com/reader/
If you are using Apple IOS, download the
Adobe Reader app from the App Store.
To configure Windows to open PDFs with Reader or Acrobat,
do the following:
- Right-click the PDF, and then choose
Open With > Choose default program (or "Choose another
app" in Windows 10).
- Choose Adobe Reader or Adobe Acrobat in
the list of programs, and then do the following: ...
- Click OK.
